For periods when the box is not being used, is there a cover plate to protect and hide the connections?

Thank you for your inquiry. To properly cover the additional openings, you will need to install either low-voltage brackets or an outlet box so that a blank cover can be securely attached.

It looks like it isn't perfectly flush with the wall with the cover, but is it flush enough to use the Samsung frame tv pro which sits about a 1/4 inch off the wall, or will it stick out more than a 1/4 inch and make the TV less flush?

The TV3LVKITWCC2 supports surface-mount installation and, like traditional wall plates, does not sit perfectly flush/recess into the wall.

Can the two outer cutouts be used for line voltage receptacles?

Thank you for your inquiry. Yes, the outer openings can be used for line voltage applications. Please review the following instructions: A P&S old work electrical box must be used. P116W, S116W, M118W & S118W are recommended for best fit. Check for clearances before installation to ensure proper fit. Center opening on TV3W may not be suitable for line voltage in all applications. (2x4 wall construction)

Can this be installed on a poured concrete wall?

They are wrong… the dimensions this previous answer gives you are the overall dimensions NOT the cutout dimensions, none of the literature provided gives you the correct information, you HAVE to have the product in your hands in order to install it correctly.
