I am installing this box in a polished concrete floor. do I need a floorport series cover? They state that they're for tile or carpet, so it's not clear if I need anything. I have a RFBA2R25 box with two device plates.
Hello. This box does take a FloorPort series cover. The flanged cover can be installed on the surface of a polished concrete floor; it can also be installed flush in polished concrete, using the FP-CTR Bare Concrete and Terrazzo ring. Thanks!

I am installing a floor box. The finished floor will be carpet. If i won't be needing to use the provided collar, does that mean the concrete will flush with the edge of my top plate ? And if so, the carpet can be installed directly over the top plate ?

My facility has RFB4-SS floor boxes set in the concrete. The doors are broken/lost. Can I purchase and use covers from the RFBA 4 gang series on the older style boxes that they replaced?

If it is just the box cover you need to replace, then yes that box will take the same box covers that go with the RFBA boxes.

I would like to know if the RFBA box (above grade) is UL-listed to be in contact with bare concrete.

The RFBA series above-grade floor box is UL listed for concrete installation and is intended to be poured in place where the slab does not come in contact with the earth (second story or higher). In addition, we offer a separate trim ring which can be added to the box prior to the pour when the floor finish is bare concrete.
