I just purchased a WP1220-WH for my in wall security camera dvr mouse but it's not working, I hooked up with a single cable, usb A male at dvr and usb A at wall j-box to WP1220-WH. Could it be defective or should I have ordered a different type?

As long as you are connecting only USB2.0 devices to that device you should be fine. USB has a finite distance that power and the signal can travel. This distance changes depending on the device(s) used. If the length isn't particularly long, then the keystone may be defective.
